INSURANCE CLAIM FILING:

For your benefit we file all insurance claims to all primary and secondary carriers. We participate in most insurance programs. Plans change frequently, please contact our office to verify participation in your plan. All co-pays are due at the time of service. We accept Visa, MasterCard, and Discover credit cards. It is your responsibility to call your insurance company to verify coverage.

PATIENTS WITHOUT INSURANCE:

Patients without insurance will be counseled at the time of their first appointment regarding payment of their bill. A first payment of $50.00 is requested prior to being seen by the physician. Once the total changes are known, a monthly payment plan can be arranged.

DISABILITY:

Our office will assist you with disability forms. Please fill out the patient portion and sign the Authorization to Release Information. There will be an $8.00 charge to complete each of the forms.

INSURANCE NOTIFICATIONS:

Most insurance companies require notification for hospital admission, inpatient, and outpatient surgeries. Please, remember it is your responsibility to notify your insurance company. Our nursing staff will assist you if necessary. Failure to notify your insurance company may result in a reduction or denial of benefits.
